Does Creatine Cause Hair Loss? What the Evidence Actually Shows

Twenty rugby players, three weeks, and not a single hair was ever counted. Here is what that 2009 study really found, what the 2025 trial that did measure hair showed, and why your shedding probably started somewhere else.

One study of twenty rugby players in 2009 is the entire basis for the creatine hair loss scare. It measured a hormone in blood for three weeks. It never counted a single hair.

In 2025, a trial finally did count them. Forty five resistance trained men, twelve weeks, hormones measured and hair measured directly with a trichogram and digital imaging.

Here is what each one actually found, why your shedding probably started somewhere else, and what this decision costs you either way.

Where this story actually started

In 2009, van der Merwe, Brooks and Myburgh published a study in the Clinical Journal of Sport Medicine titled "Three Weeks of Creatine Monohydrate Supplementation Affects Dihydrotestosterone to Testosterone Ratio in College-Aged Rugby Players".

Twenty college rugby players. Seven days of loading at 25 g of creatine a day, then fourteen days of maintenance at 5 g a day, against a glucose placebo.

Testosterone did not change. Dihydrotestosterone, or DHT, rose by about 56 percent after the loading week, and the ratio of DHT to testosterone rose about 36 percent, settling around 22 percent above baseline during maintenance.

DHT is the hormone involved in male pattern baldness. So the inference wrote itself: creatine raises DHT, DHT causes baldness, therefore creatine causes baldness.

The part nobody quotes

The study did not look at hair

No hair counts, no scalp examination, no photographs, no shedding measurement. It was a hormone study. Whether any of those men lost a single hair was never recorded, because it was never an outcome. Everything built on top of it since is inference, not finding.

Three other things are worth knowing about it. It had twenty participants, which is small. It ran three weeks, which is very short next to a process that unfolds over years. And the loading dose of 25 g a day is five times what anyone actually takes as a maintenance dose.

None of that makes the study wrong. It makes it a narrow result that has been asked to carry far more weight than it can.

What happened when someone finally measured hair

In 2025, a randomised controlled trial published in the Journal of the International Society of Sports Nutrition set out to test the question directly. Its title is literally "Does creatine cause hair loss?".

Forty five resistance trained men aged 18 to 40, randomly assigned to 5 g a day of creatine monohydrate or 5 g of a maltodextrin placebo, for 12 weeks. Blood taken at baseline and at the end for total testosterone, free testosterone and DHT. And critically, hair itself assessed, using a trichogram and digital imaging.

The finding: no difference between the groups on any hormone or hair outcome. No change in DHT, no change in the DHT to testosterone ratio, no difference in the hair measures.

Being precise about what this does and does not settle: twelve weeks is long enough to detect a hormonal change, and long enough to detect a shift in shedding. It is short compared with how slowly pattern baldness actually progresses. So the honest statement is that no study has found creatine causing hair loss, and the one study that looked directly found nothing, rather than that creatine is proven harmless to hair forever.

Would higher DHT even do it?

Male pattern baldness is not simply a function of how much DHT is circulating. It depends on inherited sensitivity of the hair follicles on your scalp to DHT.

Two men with identical DHT can have completely different hair, because one has follicles that respond to it and the other does not. That sensitivity is genetic, and it is why baldness runs in families rather than tracking hormone levels across the population.

So even taking the 2009 result at face value, a short term rise in DHT is not the same as a mechanism for hair loss. It is one input into a system whose main variable is something you inherited.

So why did your hair start falling?

Because something happened two to three months ago.

Hair grows in cycles. When something disrupts the cycle, a batch of hairs moves early into the resting phase, and they do not fall out immediately. They fall out roughly two to three months later, which is why people almost always blame the wrong thing. You blame what you started recently. The cause is usually what happened last season.

Common triggers in that window: a fever or illness, a sharp drop in calories or a crash diet, a stressful few months, poor sleep, low iron, thyroid problems, a new medication, or a big jump in training load. Several of those overlap neatly with the period when someone starts taking supplements seriously, which is exactly why creatine gets the blame.

A dermatologist can settle in one appointment what months of reading will not. They can also check for the boring, treatable causes, which is the reason to go early rather than late.

Should you take creatine if you are already thinning?

This is the honest bit, and most pages dodge it.

The evidence does not show creatine causing hair loss, and the only trial that measured hair directly found no difference. That is a genuinely reassuring result. It is also one trial, in 45 men, over 12 weeks.

If you are already losing hair and that uncertainty would bother you, that is a reasonable reason to talk to a dermatologist before starting rather than to read another article. Nobody can offer you certainty that does not exist yet, and a supplement is not worth anxiety.

What to actually check on the tub

Since the hair question does not have a product answer, here is the one that does.

Every creatine product we track in India is creatine monohydrate. All twelve of them. That is the form with by far the most research behind it, and it is what both studies above used. The premium forms sold as HCl, buffered or ethyl ester generally cost more without stronger evidence.

So the shelf is simpler than it looks. What is actually different between those twelve tubs is the price.

What creatine costs you a day

A 3 g daily dose across the twelve creatine products we track works out between 6.67 rupees and 17.11 rupees a day. Over a year that is roughly 2,435 rupees at the cheapest and 6,245 at the dearest, for the identical molecule at the identical dose.

Two things follow. Since it is the same compound, paying the higher number buys packaging and brand rather than anything you will feel. And since even the expensive end is under twenty rupees a day, cost is not the thing that should decide this either way.

The bottom line

The creatine hair loss story comes from one small hormone study that never examined hair, at a dose nobody takes long term, in twenty men, over three weeks.

When researchers finally measured hair directly, over 12 weeks at the normal 5 g dose, they found no difference between creatine and placebo. That is the best evidence available, and it points clearly.

If your hair is thinning, the useful move is a dermatologist and a look at what happened two to three months ago, not dropping a supplement that costs seven rupees a day and has nothing to do with it.

Common questions

No study has shown that it does. The scare traces to a 2009 study of 20 college rugby players that measured hormones and never examined hair. In 2025 a 12 week randomised controlled trial in 45 resistance trained men measured both hormones and actual hair, using a trichogram and digital imaging, and found no difference between creatine and placebo on any hormone or hair outcome.

One study found it did. In the 2009 trial, dihydrotestosterone rose about 56 percent after a week of loading at 25 g a day, while testosterone was unchanged, and the DHT to testosterone ratio stayed elevated during maintenance. That result has not been consistently reproduced. The 2025 trial, using the standard 5 g a day dose, found no change in DHT compared with placebo.

If creatine was not the cause, stopping will not change anything. Hair shedding triggered by illness, a crash diet, stress or an iron or thyroid problem usually recovers over several months once the trigger is dealt with. Androgenetic hair loss, the common pattern baldness that runs in families, does not reverse on its own. A dermatologist can tell you which one you have.

The evidence does not show creatine causing or accelerating hair loss, including the 2025 trial that measured hair directly. That trial ran 12 weeks, which is long enough to detect a hormonal effect but short compared with how slowly pattern baldness progresses. If you are already thinning and want certainty nobody currently has, that is a fair reason to speak to a dermatologist before starting.

Shedding that starts within weeks of a new supplement usually has a different cause, because hair typically sheds two to three months after the event that triggered it. Look at what happened in the preceding months: illness, fever, a sharp calorie cut, poor sleep, high stress, or a new training block. See a dermatologist rather than guessing, and mention everything you take.

There is no evidence that it does. The two studies most often cited on this question were both conducted in men, so the direct evidence in women is thinner still. Hair loss in women has its own common causes, including iron deficiency, thyroid problems and hormonal conditions, which are worth investigating rather than assuming a supplement is responsible.

That is a question for the doctor who prescribed them, and it is worth asking rather than assuming. Bring the specific product and dose to the appointment. Do not stop or change a prescribed hair loss treatment because of anything you read online, including this page.
